The 27-year-old Thomas crossed the finish line in 21.83 seconds while Alfred, who claimed Saint Lucia's first Olympic medal when she won the 100m on Saturday, was a clear second in 22.08.
Brittany Brown made sure there would be two American athletes on the podium, taking the bronze medal in a time of 20.20.
British duo Dina Asher-Smith and Daryll Neita finished fourth and fifth respectively.
It was a maiden global title for Thomas who won a bronze medal at the last Olympic Games in Tokyo and took silver at the world championships last year.
The Harvard graduate set herself up for a big 200m performance in Paris with an incredible year of training and racing while battling back from a hamstring injury in 2022.
She won the 2024 New York City Grand Prix in 22.42 and then in June posted the fastest time of any sprinter this year with a 21.78 at the U.S. Olympic Trials.
Just before the Olympics, Thomas won the 2024 London Diamond League in 21.82.
