The home side were all out in the first session after wicketkeeper Kyle Verreynne struck a brilliant unbeaten 105 after his team resumed the day on 269-7.
Verreynne was standing at the non-strikers end and watched as Keshav Maharaj fell early for a duck but then combined with Kagiso Rabada (23) for a vital 56-run stand.
Last man Dane Paterson then kept Verreynne as the gloveman reached his third Test century, his first in South Africa, to push the Proteas over the 350 mark.
Sri Lanka started their response with a 41-run partnership between openers Pathum Nissanka and Dimuth Karunaratne before Rabada made the breakthrough, removing the latter for 20.
Nissanka and Dinesh Chandimal then added 109 runs for the second wicket before Paterson got the latter to edge behind for 44.
All the while Nissanka, who survived a dropped chanced at first slip on 29, was going about his business and looked nailed on to reach a hundred.
The opener though lost his head in the final session when he was bowled by spinner Maharaj for 89 looking for a big shot.
Angelo Mathews (40 not out) and Kamindu Mendis (30 not out) will resume for the tourists who are still 116 runs behind the home side's first innings total.
