PSG defeated Liverpool 4-1 on penalties at Anfield, after a 1-0 victory in the second leg, sending them through to the quarterfinals.
Alexander-Arnold, 26, was substituted in the 73rd minute, with reports indicating that he sustained an ankle injury.
"(Trent) had to come off, and that's never a good sign," Slot said. "From what I've heard from people who saw the images or knew how he got injured, it didn't look good. So, I'd be surprised if he is available for Sunday."
Centre-back Ibrahima Konate also limped off during the second half of extra time, but Slot suggested that it was likely due to fatigue.
The potential absence of Alexander-Arnold is also a blow for England, who will face Albania on March 21 and Latvia three days later in the 2026 World Cup qualifiers.
Despite the disappointment of exiting the Champions League, Slot found some consolation in the fact that Liverpool were eliminated by an outstanding PSG team in what he described as an unforgettable two-legged tie.
Calling the encounter "the best game of football I've ever been involved in," Slot reflected on the team's penalty shootout loss, which saw PSG win 4-1 and progress to the quarterfinals after Dembele's decisive goal in Tuesday's second-leg match.
"Yeah, of course, it's a shock," Slot said. "But if you have to go out, and you go out like we did against one of the best teams in Europe and make such a fight out of it, I think every fan around the world was hoping the game just wouldn't stop because it was incredible."
