World number three Medvedev, who fought back from a set down against Grigor Dimitrov on Thursday, was forced to dig deep again against eighth seed Khachanov before producing some of his best tennis in the final set.
Medvedev said in his post-match interview: "It was a very close match, so of course mentally you have to be strong and it was a long one.
"But at the end, one break each set. So I'm happy that I managed to make two and he only made one."
Fourth seed Tsitsipas was also in top form as he overcame Croatian wild card Borna Gojo 7-6 (4) 7-5 in one hour and 51 minutes.
Russia's world number five Andrey Rublev sealed a personal-best 53rd win of the year by beating Alexander Zverev 6-1 6-7 (5) 6-3 and will face Jannik Sinner in the other semi-final.
Italian world number four Sinner defeated American Frances Tiafoe 6-3 6-4.
