Dortmund had been poised to unveil Guirassy as their new signing from VfB Stuttgart on Wednesday.
However, the move was thrown into doubt when Guirassy's medical examination revealed a knee injury he sustained while playing for the Guinea national team last month.
Reports suggested that Guirassy was contending with an old knee issue. Despite this, Dortmund's medical team believed it could be managed conservatively.
A specialist is set to conduct a further examination next week to assess the extent of the 28-year-old's injury and determine if he can join the squad.
In the interim, Guirassy has returned to Stuttgart, with both clubs maintaining communication on the best course of action.
German media reported that no definitive decision would be made before next week at the earliest.
Dortmund initially pursued Guirassy following his exceptional season with Stuttgart, where he netted 30 goals in 30 appearances.
His impressive performance last season was instrumental in securing Stuttgart's second-place finish in the Bundesliga. Despite interest from several major clubs, Guirassy chose to join the Black and Yellows.
Should the transfer be finalized, Guirassy will compete with Niclas Fullkrug for a spot in Dortmund's starting lineup.
